QUESTION(S) PRESENTED 1. I have genital herpes. I experience painful ulcers that appears in the open end of sores that are extremely painful. ‘Also, herpetic lesions, systemic symptoms, prodromal | symptoms and recurrent sufferings of these symptoms that do not show in the open and of extragenital lesions, these pains is excessive and daily while I suffer. Severe mental illness and denied medication, long-term treatment plan after it was approved to me and health care and ' mental health officials never studied my mental health history and reports to make professional decisions. I have explained to the Northern District Court and the Second Circuit Court of Appeals that each officials, (defendants) did not give me screening, counseling, ; medication nor any concern is given to me. 2. Ihave had the same experience with a thyroid disorder when the Southport Facility doctors said thyroid blood test is true. I continued suffering thyroid disorder and they . continued to not give me something for severe excessive pains which also affected my severe mental illness and with the genital herpes made severe excessive pains worse with the mental illness. Why the Courts did not approve me physical and mental trauma compensatory damages from documentary evidence that I sent to them? Which included ambulatory health records is of both symptoms. 3. I was denied discovery motion needed to prove the civil action and Appeals Court I was forced to suffer for many years and without guidance or evidence. I am explaining to you clear facts because my severe illness causes me to become violent. I was viciously assaulted at over 14 prisons by correction officers and sergeants. I suffer from: Delusional’ Disorder; Psychotic Disorder; Major Depression; Depressed Mood; Psychotic Behavior; Paranoid Thoughts; Emotional Stress; Anxiety; Anti-Social Personality Disorder; Stress; Not Sleeping; ' Impulsive Behavior; Assaulting Behavior; Constant Hunger Strikes and; Chronic History of Throwing Feces and Urine. I do not have money for mental health history records, health care records and disciplinary history report records. Upstate Correctional Facility does not have all my records and no one at the N.Y.S. Department of Corrections is sending it to me. My illness is worse and only the Courts can provide me help of enforce it. How will I be able to fight or prove I speak true facts and in writing? From 1994, my Genital Herpes and Thyroid Disorder cause me excessive pain. 1994 and 1995 medical records at Franklin Correctional Facility 7 proves it. I still suffer pains from it and health care service officials never gave me medical care for it at any facility and Doctor Kumar said it’s at normal level but I put in for sick-call he knows severe pains last up to 3 or 4 weeks he always make appointment one week after or one month after or 3 or 4 weeks suffer with pains is over. 4. These severe illness I was given criminal charges ay majority facilities when every cause of assault on officers is documented while suffering mental illness or when I was denied treatment and medication approved to have.
